ASSESSMENT FORMS
Includes instructions and guidelines for assessing the student’s progress, or assessment for specific services.
PRIOR WRITTEN NOTICE (PWN)
The PWN informs parents of changes to their child’s special education program.
SPECIFIC LEARNING DISABILITY FORMS
These forms are documents that helps determine if a student is eligible for special education services.
TRANSITION FORMS
Outlines their post-graduation goals and the necessary steps to achieve them, essentially acting as a roadmap to prepare them for life after high school, including areas like education, employment, and independent living skills.
ESY ELIGIBILITY WORKSHEET
A document used to determine the services a student with special needs will receive during the summer months.
EMERGENCY CIRCUMSTANCE
A document used to outline how a student with disabilities will receive their educational services during a temporary emergency situation that disrupts normal school operations, such as a natural disaster, pandemic, or major infrastructure issue, preventing them from attending school in person; essentially, it’s a plan to ensure continued access to appropriate education despite the emergency.
POST SECONDARY EXIT PAGES
Is an exit IEP, or Individualized Education Program, that a student receives when they graduate high school or age out of their school district’s services. The exit IEP is a culmination of the student’s transition plan and is used to prepare them for life after high school.
MANIFESTATION
This document is used to initiate a “Manifestation Determination Review” (MDR), which is a process to determine if a student with a disability’s disciplinary behavior is directly related to their disability, meaning it is a “manifestation” of their disability, and therefore cannot be punished in the same way as a non-disabled student would be; this review typically occurs when a student with an IEP faces significant disciplinary action like suspension or expulsion.
BEHAVIOR INTERVENTION PLAN
A document that outlines specific strategies and interventions designed to address a student’s challenging behaviors.
INTERIM PLACEMENT
The interim placement form in special education allows a student’s previous Individualized Education Program (IEP) to continue for 30 days. The form is used when a student is placed in an Interim Alternative Educational Setting (IAES).
REFERRAL FORMS
The referral form is a request for an evaluation for a student who may need special education services.
EL RECLASSIFICATION
Is a document used by schools to determine if a student previously identified as an “English Learner” (EL) has gained sufficient English proficiency to be reclassified as “Fluent English Proficient” (RFEP), meaning they no longer require specialized English language development support.
