The 504 team is a group of individuals responsible for developing, reviewing, and updating a student’s 504 Plan. The 504 team collaborates to create a plan that supports the student’s educational success and ensures they receive the accommodations they need.

WHO MAKES UP THE 504 TEAM?
PARENTS/GUARDIANS
As parents/guardians, you know the student best! Your input on student’s strengths, needs, likes, dislikes, favorite activities, behavior at home, and goals are invaluable!
GENERAL EDUCATION TEACHER
General education teachers are essential as they have direct knowledge of the student’s academic performance and classroom behavior. The general education teacher helps ensure the 504 supports success in the classroom.
SPECIAL EDUCATION TEACHER
This person provides expertise in special education strategies and accommodations.
SCHOOL DISTRICT REPRESENTATIVE
School principals or other administrators, often including the 504 coordinator, ensure that the school’s policies and procedures are followed.
SCHOOL COUNSELOR/GUIDANCE COUNSELOR
School counselors can provide valuable information about the student’s emotional and social needs, and can help with developing appropriate accommodations.
EVALUATION SPECIALISTS (IF NEEDED)
Depending on the student’s needs, other specialists like psychologists, therapists, or other relevant professionals may also be involved.
SCHOOL NURSE
If the student’s disability is related to a medical condition, the school nurse can provide expertise and support.

In some cases, older students may be included in the 504 team meetings to share their perspectives and needs.
