FINALIZING THE 504 PLAN
The school team will finalize the 504 based on what was discussed in the meeting. Parents should receive a copy of the finalized 504 Plan for review and signature.
IMPLEMENTATION OF ACCOMMODATIONS AND/OR SERVICES
Once you have the 504 Plan in place, it’s important to keep track of your child’s progress. What you want to know is your child consistently getting all the accommodations outlined in the 504 plan? Are the accommodations helping your child to make progress in school and take part in other school activities? Does your child have any new challenges that should be evaluated?
PROGRESS MONITORING
The school tracks the student’s progress toward their IEP goals and provides updates to parents. Progress reports are often shared during report card periods or at agreed-upon intervals.
FOLLOW UP & COMMUNICATION
Parents and teachers should maintain open communication about how the 504 is working. If issues arise, parents can request additional meetings to discuss adjustments.
REVIEW & REEVALUATION
If you think the 504 Plan needs to be changed, talk with your child’s teachers and the 504 Coordinator.
WHAT HAPPENS IF YOU DON'T AGREE
